Acknowledgement of Risk
I, the undersigned, acknowledge that participation in Pilates, Barre, Yoga, Reformer, and other physical fitness activities at The Core Space involves physical exertion and carries inherent risks. These may include, but are not limited to, muscle strain, sprain, falls, or equipment-related injuries.
I understand that such risks can occur despite proper supervision and safety precautions.
Health & Medical Declaration
I affirm that I am physically fit to participate in physical activities and that I have not been advised by a medical professional to avoid such exercise. I am solely responsible for monitoring my own condition during classes. I understand that The Core Space strongly recommends consulting a physician before beginning any new exercise program, especially if I have any pre-existing medical conditions, injuries, or concerns.
Voluntary Participation
I acknowledge that my participation is completely voluntary and at my own discretion and risk.
Assumption of Risk
I fully understand and voluntarily accept all risks—known and unknown, foreseeable or unforeseeable—associated with my participation in any activity, whether it is supervised or unsupervised, and whether it involves use of equipment or studio facilities.
Release of Liability
In consideration for being allowed to participate in classes, workshops, or use the facilities at The Core Space, I hereby release, waive, and discharge The Core Space, its owners, instructors, employees, affiliates, and representatives from any and all liability, claims, or demands of any kind, whether caused by negligence or otherwise, arising out of or related to any injury, loss, or damage I may experience in connection with my participation.
Indemnity Clause I agree to indemnify, defend, and hold harmless The Core Space and its staff from any claims, damages, liabilities, costs, or expenses (including legal fees) resulting from my participation or conduct.
Media Release (Optional)
I grant permission to The Core Space to use photographs, videos, or other media captured during classes or events for promotional and marketing purposes.
I understand I may opt out by informing the studio in writing.
Minors (if applicable)
If this waiver is being signed on behalf of a minor under the age of 18, I confirm that I am their parent or legal guardian, and I agree to the terms of this waiver on their behalf.
Severability
If any provision of this agreement is held to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
Governing Law
This agreement shall be governed by and construed in accordance with the laws of the Republic of the Philippines.
